You can check each item as you review it. These marks are not sent to IHR and do not confirm an appointment.

• Confirm your visit by phoneYour appointment must be confirmed by phone at least 48 hours before the requested time. If you have not received confirmation, call before traveling.
• Ask what to bringAsk the team which identification or documents are needed for your visit. Keep your request reference handy. Do not send medical records through the public website forms.
• Receive your specific instructionsAsk how long the visit may take and whether it needs special preparation. Follow the instructions given by the clinical team for your visit.
• Tell us how we can helpConfirm your preferred language and ask about access needs, parking and whether a companion can attend. If you need transportation assistance, ask whether any arrangement is available for your study.

Questions worth bringing

• What is the study trying to learn, and what would I do?
• What risks, possible benefits and other options should I consider?
• How many visits are involved, and how will they affect my schedule?
• Which costs, reimbursements or payments apply, and who can answer questions afterward?

Participation is voluntary. Take time to understand the study and ask questions before deciding. Requesting or attending a visit does not establish eligibility or enrollment.

Call 305-927-5049 and mention your request reference, if you have it. Ask the team to arrange a different date; changes must be confirmed.
